Below are the required forms for admission to the upper school:
1. Immunizations: must be submitted at registration from a licensed health care provider providing documented proof of completed immunizations as required by NYS law.
The following are changes to the New York State immunization requirements for the 2019-2020 school year:

2. Physicals: All 9th, 11th, and new transferring students are required to have a physical examination by their private physician submitted by August 1st.
3. Student Health History Form- completed by parent
4. Residency Form- ONLY to be completed by residents living in the Bronx and NYC, including a recent utility bill (ie: Con Ed, Phone/Cable, Water).
All health forms can be downloaded in the FORMS section to the right. They may be mailed, faxed or handed in by student.
Medications may be administered in school, see forms below. For your child to receive any medication in school, over-the-counter or prescription, a written order from a physician and written consent from the parent must be on file in the health office. Orders are valid for 12 months, and the medication must be in the original container and properly labeled.
There is a self administration form available for students who need to have medication with them at all times, ex. an Albuteral inhaler.
